Worker's Comp: Request an Appointment

If you are requesting an appointment for a Worker's Comp injury please take the following steps.

1) Call 954-473-6344 to make an appointment, or make an appointment online. Be sure to let us know this is a Worker's Comp injury.

2) Print and complete the Worker's Compensation Patient Information Form. Bring the form with you to your appointment.
